Who Should Be Referred for Hospice or Palliative Care?

1. For hospice or palliative care:  those residing in the Hospice & Palliative Care Charlotte Region service area (Mecklenburg, Iredell, Cabarrus, Union, Lincoln, Gaston, Cleveland, Catawba and York counties)
   
2. For hospice care:  patients referred and followed by their physician or healthcare provider, and who have a life expectancy of six months or less.  Referrals may also be made by patients or family members.
   
3. For palliative care:  Individuals desiring palliative care with a focus on comfort care and supportive interventions.
   
4. Those who live alone must desginate a responsible person to coordinate health care when necessary.
   
5. For hospice care:  those who live in assisted living residence or who are in a skilled nursing facility and require end-of-life care.
